This session will focus on working with at risk veteran families (i.e., homeless/older/mental health challenges, etc.). Using real life case studies, this interactive training will review SOPs from initial intake, creating a service plan, identifying and leveraging resources to address social determinants of health, and follow up. As part of this training, participants will learn how to access and use PATRIOTlink to identify additional resources for their clients.
